依安县暴发肾综合征出血热原因探讨

摘要: 本县1986年暴发肾综合征出血热,共发病508例,发病率为108.55/10万;病死23例,病死率为4.53%。全县共18个乡镇,有17个发病,占94.44%。病例呈散发,又相对集中,有4个乡镇的病例数占总病例数的78.54%。发病时间、年龄、性别等与历史资料一致。全年病例数与总鼠密度不相关(r=0.2314,n=9,P>0.1),而与黑线姬鼠密度及构成比显著相关(r=0.7634,n=9,P<0.005),即当黑线姬鼠构成比达40%以上时,必有病例发生;发病以l1月份最高,黑线姬鼠构成比高达71.81%。由此可见,该鼠是本病暴发流行的主要宿主动物和传染源。

Abstract: An outbreak of HFRS occurred in Yi An County,Heilongjiang Province in 1986,A total of 508 cases occurred,with a morbidity of 108.58/100000, and 23 casesdied, with a case fatality rate of 4.53%.There were 18 villages and towns in this County, of which 17( 94.44%) had occurrence of this disease. Although cases occurred sporadically, yet relatively concentrated.The number of cases in 4 villages and towns accounted for 78.54% of the total.The time of occurrence, age and sex were consistent with the data recorded in history.Two hundred and ninety patients were male with a morbidity of 120.83/100000;218 patients were female with a morbidity of 96.18/100000.The ratio of male to female was 1.33:1; the ratio of morbidity was 1.26:1.This was significantly different from the ratio(2.99:1) in data recorded in history of Heilongjiang Province(X2=1784.63, P<0.005, X2=3219,P<0.005). The number of cases in the whole year had no relation with the density of rats(r=0.2314,n=9, P>0.1). However, it had relation with the density and constitutional percentage of Apodemus agrarius(r=0.7634,n=9, P>0.005).When the constitutional percentage of Apodemus agrarius reached 40% or above,appearance of cases happened. The peak of the curve of this disease occurred in November, and the constitutional percentage of Apodemus agrarius reached 71.81%.Apodemus agra-rius was the main source of infection.
