武汉市2002~2003年大肠埃希菌O157∶H7监测情况

Study on Diarrhea Disease Caused by Enterohemorrhagic Escherichia coli O157∶H7 in Wuhan City in 2002-2003

摘要: 目的了解武汉市大肠埃希菌O157∶H7在腹泻病人中的检出情况和宿主动物带菌及毒力基因情况。方法根据广覆盖的原则,选择8家大型综合医院作为O157∶H7监测点,监测腹泻病人的带菌情况;其中2家医院的肾病内科监测可疑肾衰病人,选择3家养殖场,采集猪、奶牛、鸡的粪便标本,采集生猪肉、奶制品等,用mEC培养基进行病原菌分离培养,用“金卡”作初筛,用聚合酶链反应(PCR)进行毒力基因分析。结果8家监测点共监测腹泻病人858例,肾病内科肾衰病人6例,未检出大肠埃希菌O157∶H7或O157∶H-;3个宿主动物监测点共采集奶牛、猪、鸡的粪便标本318份,检出大肠埃希菌O157∶H- 8株,带菌率为2.52%,其中猪的带菌率最高为5.88%,奶牛为1.96%,鸡未检出;食品类生猪肉中检出1株O157∶H-,检出率为1.00%;对9株菌株进行O抗原PCR和H抗原PCR鉴定,O抗原均为阳性,H抗原为阴性;单克隆血清抗体6株阳性;并进行毒力基因鉴定,均为阴性。结论武汉市到目前为止尚未发现O157∶H7病人,猪和奶牛是主要宿主,其带菌率没有明显差异,生猪带菌率有明显的上升,且从生猪肉中检出1株O157∶H-,尚未检出带病毒基因的菌株;而武汉市居民以食用猪肉为主,值得密切关注。提示加强宿主动物和食品大肠埃希菌O157:H7监测,特别是生猪的监测,结合腹泻病人的监测,对控制大肠埃希菌O157:H7的流行有重要意义。

Abstract: Objective To understand the Escherichia coli O157∶H7 carrier rate of host animals and the toxic gene of the strains in Wuhan city.Methods Surveillance spots were set up,to surveillance Escherichia coli O157∶H7 carrier status of patients;and to collect feces of pigs,cattle,chickens,and samples of pork,milk products to culture for O157∶H7 with mEC.Toxic gene of the strains with PCR were detected.Results No O157∶H7 or O157∶H- were separated from 858 patients in eight hospitals.Eight strains of O157∶H- were separated from 318 feces of different animals in three spots,with a overall positive rate 2.52%,the positive rates of pig,cattle and chickens were 5.88%,1.96% and 0 respectively.One strain of O157∶H- were separated from 99 samples of pork,the positive rates of edible(pig) were 1.00%.The 9 strains were shadow with toxic genes were detected.Conclusion Patients with O157:H7 were not discovered so far.Pigs and cattle were main host animals of O157∶H7.The datum indicated that it was important to enforce the surveillance of Escherichia coli O157∶H7 for better control of the disease in Wuhan city.
