酶免疫斑点试验用于钩端螺旋体病快速诊断及疫区传染源调查的研究

摘要: 应用酶免疫斑点试验和显微镜凝集试验,对四川、云南和湖南3省临床确诊的不同临床期钩体病人174份血清进行了比较研究,前法阳性率(四川36.67%,云南54.88%,湖南79.03%)明显高于后法(四川10.00%,云南25.37%,湖南62.90%)。发病后采血时间越早,两法阳性率差别越大,总符合率越低,反之越高(四川73.33%,云南90.25%,湖南67.74%)。前法的敏感度为86.27%,特异度为96.77%,可用于钩体病的快速及早期诊断。我们还对江西钩体病疫区14头牛和7头猪进行了检测,结果前法的阳性率(71.43%)也明显高于后法(14.29%),说明前法也适用于钩体病疫区传染源的调查。

Abstract: The dot enzyme linked immunosorbent assay(Dot-ELISA) was Compared to the microscopic agglutination(MAT) and the blood Culture. One-hundred and Seventy-four Serum Samples from patients with clinical diagnosis of early Leptospirosis who came from the Sichuan、yunnan and Hunan provinces were tested by Dot-ELISA,MAT and blood culture. The results appeared that the positive rates of Dot-ELISA (Sichuan 36. 67%, yunnan 54. 88% and Hunan 79. 03%)were higher than that of MAT(Sichuan 10. 00% yunnan 25. 37% and Hunan 62. 90%)(x2=8.74,P<0.025) and blood culture(Sichuan 3.33%); The earlier the time of the illness, the higher the positive rates of the Dot-ELISA than that of MAT; The Sensitivity of the Dot-ELISA was 86. 27%,the specificity of the Dot-ELISA was 96. 77%; The positive Correlation was observed between the Dot-ELISA and MAT, the Correlation Coefficient was 0. 9879; The earlier the time of the illness, the lower the accordance rates between the Dot-ELISA and MAT(Sichuan 73. 33%, yunnan 90. 25% and Hunan 67.74%). Fourteen Serum Samples from cattles and buffalo and Seven Serum Samples from pigs that came from infectious area in Jiangxi province were tested by Dot-ELISA, MAT, blood culture and urine culture, the positive rates of Dot-ELISA(71. 43%)was higher than that of MAT (14. 29%)and urine culture(11.11%). The Dot-ELISA appeared to be Sensitive and specific for the Serodiagnosis of early and acute Leptospirosis and infectious resources investigation. In addition, this rapid test is inexpensive, simple to perform, utilized minute volumes of killed Leptospiroais antigen and is easily daptable to field use.
